Equipment Request Form


Click on the 'Download Form' button below to access the Equipment Request Form. This form is a fairly large file and may take up to a couple of minutes to download to your computer.
Full training on the requested equipment must be completed in order to make a reservation. Requests may be submitted via fax or email. The individual making the request must pick up and drop off the equipment personally. Photo ID and a signature will be required upon picking up the equipment. Anyone under the age of eighteen must be accompanied by a parent or legal guardian when picking up and returning equipment. A finished program must be submitted for broadcast on GO-TV no later than eight weeks after the equipment has been returned. All projects videotaped with GO-TV equipment must air on GO-TV prior to any other cablecast.
