Producer Access Agreement Form

If the "producer" (the person responsible for the production of a program) is eligible to participate as a "GO-TV Producer" (lives, works, volunteers for an organization in the Town or Village of Ossining), then the producer must complete the Producer Access Agreement and submit the form with each non-series program or with the first episode of a series program in order to receive a time-slot for broadcast. If the "producer" is not eligible to participate as a "GO-TV Producer", the "local sponsor" (person who lives, works, volunteers for an organization in the Town or Village of Ossining and is willing to take responsibility for the program) must complete the Producer Access Agreement and submit the form with each non-series program or with the first episode of a series program in order to receive a time-slot for broadcast.
